Emphasis is on direct patient care, working in coordination with the staff surgeon, house staff, hospital staff, and the patient for the following:

6. Performs invasive procedures such as placement of arterial and central venous lines, ventricular and lumbar drains and other invasive . 
7. Provides first line care for neurosurgical patients in critical care environment as well as on inpatient units
8. Dictates discharge summaries and transfers orders.
9. Participates in patient rounds and case conferences.
10. Maintains and follows patient care protocols.
11. Works in collaboration with staff surgeon and house staff in educating the patients and their families in medical care.
12. Works with staff in providing patient care in the following locations:  the intensive care units ( SICU, MICU), Neurosurgical Nursing Units, PCU,  and staff  neurosurgeon's clinical practice area.

Oversees the schedule, vacation and workload of the other Clinical Assistants. Performs other duties as requested or required.

Unsupervised Procedures: Insertion and removal of chest tubes (thoracostomy) Hemodialysis access Insertion and removal of central line placements Insertion and removal of arterial lines and Swan ganz floats Troubleshoot intra-aortic balloon pump catheters Remove intra-aortic balloon catheters Remove temporary pacing wires (which are implanted in chest) Initiate blood products for hemodynamically unstable patients Dictate discharge summaries Diagnose Prescribe treatments/medications of controlled substances for Schedule II through VI History and physicals for surgery.

Duties With Supervision: Establish treatment plans for ICU and floor patients. Evaluate patients for surgery.  Coordinate pre-op studies. Pre-operative H&P for all patients. Educate post-operative patients in office.

ICU Duties Without Supervision: Minute to minute management of ICU patients and floor patients.  Includes emergent evaluation and treatments as follows: Hemodynamic Instability-Initiating and titrating inotropes and pressors including but not limited to Dopamine, Dobutamine, Levophed, Milriwone, Epinephrine, Neosynephrine, Pitressin, Nitroglycerin, Nipride, Labetolol, Esmolol. Acute Ischemia-Ordering and Interpreting EKG¿s.  Coordinate with surgeons as to emergent therapy.  All ischedmia in a post-operative cardiac surgery patient in an urgent situation.  Includes pharmacologic therapy as well as placement of IABP as needed.

Supervision of residents or independent performing the following procedures: Placement of central lines Placement of swan-Ganz Catheter Placement trans venous pacers Placement of chest tubes Placement of IABP Placement of dialysis catheters Emergent sternotomy for hemodynamic crash Wound management-debridement Vac dressing

Arrhythmias: Diagnosis and management of atrial fibrillation, atrial flutters, supraventricular tachycardia, ventricular fibrillation, 1o,2o and 3o heart block.  Need to be able to confidently administer pharmacologic therapy, defibrillate, cardiovert as well as placing temporary pacing wires and management of pacemakers.

Complex Ventilator Management: Differentiate modes of ventilation.  Complex understanding of pressures and PF, initiate ventilator weans and resuscitation of patients with respiratory compromise. Interface directly with attendings and other caregivers regarding multiple medical problems including nutrition, hemodialysis and consultative needs.

Matagorda Episcopal Health Outreach Program (MEHOP) has an opening for a Family Nurse Practitioner at its fixed site clinic.  Proposed hire date:  Immediately.

MEHOP is currently in the process of applying for a Federally Qualified Health Center –Look-a-Like (FQHC-LA) designation.  MEHOP currently provides healthcare delivery via a 36 ft. mobile medical clinic, which operates four days a week, traveling to six rural communities of Matagorda County.  Beginning in May 2005, MEHOP will expand its service delivery to a fixed site clinic that will operate four days a week. 

Matagorda County lies in the Gulf Coast approximately sixty miles southwest of Houston, Texas.  Matagorda County has an estimated population of 37,608 and is designated as a Medically Underserved Area (MUA) by the Texas Department of Health. 

MEHOP provides primary health care on an acute and chronic basis along with spiritual ministry. Services are offered to the isolated, poor, disadvantaged and to those who are otherwise unable to access medical assistance regardless of age, gender, religion, or ethnicity.  The clinic is accessible to people with disabilities.

MEHOP is currently staffed with a Family Nurse Practitioner, Registered Nurse, Administrative Assistant, Registering Clerk, and 25 Community Volunteers overseen by a Medical Director and a CEO.
